Missouri Birth Certificates Available at Heatlh Department

A new law took effect July 1, 2005 that requires all Missourians to show proof of identify, residency and that they are lawfully in the U.S. before a driver license, permit or non-driver license can be issued or renewed by the department of revenue.

To prove “lawful presence,” an individual may present to the license office, among other things, a birth certificate with an embossed or raised seal issued by a state or local government.

Certified copies of Missouri birth certificates for births from 1920 to present can be obtained at the Phelps/Maries County Health Department's main office located in the ground floor of the Phelps County Courthouse. To obtain a copy of a birth certificate, you will be required to complete an application and produce a "picture ID" or other acceptable form of identification. A fee of $15 is charged for each certificate, and they are printed in the office while you wait. Office hours are Monday through Friday 8:00 a.m.-5:00 p.m.
